A depression management dissemination study in representative primary care practices

Abstract: The trial will test an intervention for patients suffering from major depression who are seen in primary care settings. Representative practices serving diverse patient populations are the target settings. The proposed intervention is intended to be powerful in impact on patient outcomes, yet modest in demand on practices making it feasible to introduce to practices nationally through readily available external resources. Practices will have latitude in how they address the intervention components: formal assessment of depression; patient education and engagement; depression management; and patient monitoring. This is a dissemination study which is evaluating methods of changing practice environments. This includes enhancing patient adherence/ activation, providing depression education to providers, using chronic disease management systems to improve monitoring and follow-up, and helping practices identify and remove barriers that interfere with providing guideline concordant care for patients with depression. This project is in the planning phase. Research Objectives: 1. Create a "model of depression care" that facilitates patient treatment with guideline concordant care. 2. Test and describe the impact of intervention components on depression care, measured by practice uptake and patient outcomes. Research Design: The study is a randomized control trial following patients for 6 - 12 months. After baseline assessments, Intervention Practices receive the intervention components, then follow-up data are collected. Control Practices then participate in the intervention at a later stage of the project, allowing all practices to benefit from the intervention materials. Descriptive measures are used to describe the change process, the incentives and barriers that contribute to guideline concordant care and practice and patient characteristics.
