| Abstract: | Primary Care (PC) Plus is the only model of Medicaid managed care in Vermont. The goal of this project is to expand the capacity of PCPlus to provide beneficiaries who have complex needs with a "medical home" -- a place where preventive, primary and specialty health services they need are provided and coordinated in such a way that they can live successfully in the least restrictive settings within their communities. This project's specific objectives are to: develop a medical home physician training curriculum, develop community resource guides for physicians and beneficiaries, and develop performance measure profile reports. The final product of this project will be a monograph that gives recommendations to other states running primary care case management (PCCM) programs on: 1. how to find people with special health care needs, 2. how to analyze cost, mix and utilization of services for people with special health care needs, 3. how to identify and evaluate PCCM networks for people with special health care needs, 4. how to identify needed social services and develop linkages with community based organizations, and 5. PCCM models available for states. |
