Abstract: This study will examine whether or not a family focused intervention will assist patients with heart failure to maintain their health related behaviors. Specifically we will examine maintenance of a low sodium diet, exercise patterns and drug administration. The effects of the program will be examined in these aspects of patient behavior and also the family members' outcome of psychological state of depression. Heart failure is one of the most frequent reasons people over the age of 65 are admitted to the hospital, and many reasons account for readmissions, including noncompliance with prescribed regimens of care. This study will attempt to address a way to improve what could be considered a preventable reason for readmission by incorporating family support into the patient's plan of care.
