Abstract: $278,125 to Stanford University, the Palo Alto Medical Center and the University of California, San Francisco to develop a model for the management of chronic disease. Two-thirds of current health expenditures are caused by chronic disease. Patients who live with a chronic condition need to be able to assess disease trends, take substantial responsibility for self-management, identify personal preferences concerning treatment and become an active partner in decisions. This study will evaluate a model for creating a partnership between treating physician and chronic disease patient.
