Abstract: $716,523 to the University of Pennsylvania Health System in collaboration with U.S. Quality Algorithms to design, implement and evaluate a disease management program for patients with diabetes mellitus. This study will aim to develop, implement and evaluate a disease management program for the treatment of adult patients with diabetes mellitus, aimed at enhancing patient outcomes at optimal cost. In addition, the study will highlight factors related to successful outcomes, and develop hypotheses on more effective strategies for managing chronic diseases from a systematic, population-based approach.
