| Abstract: | The Center for Substance Abuse Treatment (CSAT) and other federal agencies are developing a regulatory framework for opioid substitution therapy that will change the current regulatory system for monitoring opioid treatment programs (OTPs) to an accreditation/regulatory approach. Over a five year period, CSAT will conduct and evaluate a phase-in program to obtain information that will aid in the full implementation of the new system. The evaluation will examine the processes, costs, and impacts of the new approach, and will allow the concerns regarding costs and public safety, expressed by the states, the Office of National Drug Control Policy (ONDCP), the Drug Enforcement Agency (DEA), and other stakeholders, to be addressed systematically. The evaluation will be conducted in two phases. During Phase I, which will last three years, CSAT's Office of Pharmacological and Alternative Therapies (OPAT) will award from one to three contracts to national accreditation bodies. These bodies will develop OTP accreditation standards based on guidelines provided by the government. CSAT will select up to 180 OTPs (based on the number of accrediting bodies) to undergo the accreditation process and collaborate in the evaluation. CSAT's Program Evaluation Branch (PEB) will manage the evaluation through a separate contract with an independent evaluator. During Phase 2, years 4-5, CSAT may implement any or all of three options: (1) a longer follow-up period of Phase 1OTPs; (2) a replication of Phase 1 with a second OTP cohort; or (3) up to six special studies focusing on policy or other issues that arise during Phase 1. The goal of the evaluation is to obtain information that can guide the full national implementation of the new system for OTPs through systematic study of the processes, barriers, costs associated with a change from regulatory to an accreditation/ regulatory process, and various other impacts. |
